Free backup-plan builder for family caregivers
If you had to stop caregiving today, could someone you trust take over?
Care Relay finds the gaps, identifies what to fix first, and creates a practical first-hour and first-day handoff plan—in about seven minutes, without an account or medical details.

How Care Relay works
From “what if?” to a plan someone can use.
Care Relay asks about the people, access, current sources, permissions, and daily routines another caregiver would need. It turns weak handoff links into actions someone can follow and rehearse.
01
Map the handoff
Choose the care work, backup people, access, and time window.
02
Check the sources
Rate whether trusted records and permissions are current and findable.
03
Practice the plan
Print the first-hour plan and test it with both backups.
Free, private by default, and no account required.
Build my backup planBefore you begin
Sensitive situation. Small amount of data.
Enter roles and findability—not diagnoses, medication names, credentials, codes, or identifying numbers. Answers disappear with this tab unless you knowingly save an unencrypted browser copy.
Does Care Relay store my answers?
By default, answers live only in the open browser tab. You can explicitly choose to save a copy in that browser's local storage, which is not encrypted and may be visible to other people using the same browser profile. Clearstory does not receive the answers.
Should I enter medication names or medical details?
No. Care Relay asks whether trusted sources are current and findable, not what they contain. Keep medications, diagnoses, account credentials, door codes, and identifying numbers out of the tool.
Is this an emergency or legal plan?
No. Care Relay is a planning aid, not medical advice, emergency response, a care order, or a legal document. Call 911 for immediate danger and verify medical and legal instructions with current sources and qualified professionals.
Can I use the plan without creating a Clearstory account?
Yes. You can complete, print, copy, or download the plan without signing in. Creating a Clearstory workspace is an optional next step if maintaining the records behind the handoff becomes difficult.
